Benefit Enrollment - Health Tab

Access this page using Action>Benefit Enrollment.

Use this tab to select or decline your Health plan benefits. This page lists all of the active Health Insurance plans. The Document column contains the Document Link to the provider or a web url entered on the HR Management>Maintain>Benefit Enrollment>Enrollment Plans form for this Health Insurance Enrollment Type. The Dependent Health Plan section displays for Health Options that Require Dependents. After making your and your dependents health elections, click the Next button to continue through the Open Enrollment process or Back to return to the previous tab.

Fields 

Health Plan Election

• Action: Use this area to enroll for a benefit or remove it from your Summary page. You may also elect to decline a benefit.
  • Select - Click this option to elect this benefit for enrollment. The Plan Editor form displays. Complete this form and click Save to complete your selection.
  • Remove - Click this option to remove your benefit selection from the Summary page.
  • Decline - Click this option if you do not wish to enroll. If declined, you may have to provide additional information. This option is only available if the Require Decline box is checked in the HR Management>Maintain>Benefit Enrollment>Enrollment Plans form.
• Health Plan: The system displays the name of the health benefit plan.
• Document: If available, click the link to open a web page that points to more information about this health option.
• Rate: The system displays the rate of the health option.
• Monthly Cost: The system displays your cost per month for this heath option.

Note: To see the Monthly Cost of your benefit elections, Select a Health Plan Election, on the Plan Editor select a Plan Option from the drop-down list, click Save to populate the Monthly Cost column with the associated rate. You can then go back to the same plan and select a different option to see that monthly cost.

Additional Plan Election: After you have selected your primary health plan, if applicable, this section displays for Health Options that are associated to your primary health plan, such as, a rider for Dental or Vision.

  • Remove - Click this option to remove your benefit selection from the Summary page.
